The Diversified Investment Advisors Scholarship requires the following additional documentation:

Two recommendation letters (professional, academic, or community involvement)
Official transcripts from all colleges and universities attended

Please note that the Diversified Investment Advisors Scholarship application deadline is February 26, 2010.

Application forms and all required documentation for our other 2010 scholarships (to be awarded in the spring of 2010) will be accepted until January 11, 2010.  No applications or documentation will be accepted after 5:00 p.m. CT on that date.

You may apply for more than one scholarship but must submit a separate essay for each.  Essays should not be longer than 500 words per question.

By submitting an application, you grant the Institute permission to use your application material for promotional purposes and agree to participate in promotional activities, as requested by the Institute and/or scholarship sponsors.

The application includes an on-line form and a personal statement (300-500 words) describing your interest in health care management and your career goals.  If you cannot finish your statement at the time that you apply, you can save it and return to it later, or you can compose your statement off-line and cut-and-paste it into your application.

In addition, you must provide a current resume.  The resume must be uploaded and attached to your online application.
